Patient Hands wants to provide an individualized assessment and care plan for each client we serve. We would start by doing a preliminary phone call where we would listen to your needs. Next we would schedule a home visit, in which the client would go over specifically what they are wanting with our coordinators.
After the home visit, a custom care plan will be created that will fit the client’s needs and budget. Patient Hands Coordinators can also go over insurance information with you to determine the best method of payment for you.
